Welcome to the West Springfield Adult Day Health Center!

Our Adult Day Health program offers individuals a welcoming place to socialize, make new friends, enjoy activities and community outings, as well as receive necessary health and medication monitoring. We provide an environment that ensures not only the safety of each participant, but also keeps the older person active, alert, and aware.

The West Springfield Center is Hawthorn’s largest Adult Day Health site. The center has the ability to be split into two almost totally separate service areas, allowing for participants of very different activity level and interests to be served. This center features an Alzheimer’s/Dementia program for individuals who may require more monitoring and supervision. The center is handicap-accessible and has four bathrooms, as well as a kitchen area and smaller rooms for quiet space and private meetings. The outdoor area features a large raised garden bed for the popular participant garden club.
