The Outreach Program

The Outreach Program serves adults, referred by the Department of Mental Health, who live independently in the community. The aim of the program is to assist individuals in remaining an active part of the community and to maintain the highest degree of independence possible.

Hawthorn believes in a minimalist approach in assisting individuals in achieving this goal and work closely with each individual to provide and coordinate services. The Outreach Program provides assistance with money management, transportation, activities of daily living, medical/psychiatric concerns, crisis intervention, and socialization to name just a few.

Each individual in the program is assigned a primary nurse employed by Hawthorn to ensure individualized care. The Outreach staff work closely with other community service organizations in order to ensure comprehensive services for everyone.
