The Residential Program

Established as a viable alternative to traditional institutional care, the Residential Program features round-the-clock assistance in self-care skills, money management, medication administration, socialization, and activities of daily living. Nursing and clinical staff are available 24-hours a day, 7 days a week to offer support to direct care staff. The program provides structures for those individuals who require 24 hour assistance, but remains flexible to allow for proportional assistance to a wide range of needs and functioning levels. Staff ratios and levels of supervision are regularly adjusted to meet the individual needs of the consumers at any given time.

All of Hawthorn’s residences are built with funding provided by HUD .  The houses are fully handicapped accessible and offer a wide range of adaptive equipment with an objective to always challenge people to remain as independent as possible. Each individual consumer has his/her own private bedroom in a bright, clean, and cheerful environment. While staff support is regular and sometimes intense based on individual needs, each consumer does assume some ownership of the residence by regularly assisting with cleaning, cooking, shopping, yard work, and general “homeowner duties”.

The Outreach Program »

The Outreach Program serves adults, referred by the Department of Mental Health, who live independently in the community. The aim of the program is to assist individuals in remaining an active part of the community and to maintain the highest degree of independence possible.

Hawthorn believes in a minimalist approach in assisting individuals in achieving this goal and work closely with each individual to provide and coordinate services. The Outreach Program provides assistance with money management, transportation, activities of daily living, medical/psychiatric concerns, crisis intervention, and socialization to name just a few.

Each individual in the program is assigned a primary nurse employed by Hawthorn to ensure individualized care. The Outreach staff work closely with other community service organizations in order to ensure comprehensive services for everyone.
