Services are designed to provide social and health maintenance to adults who need supervised care in a safe place outside the home during the day. Our community based Adult Day Health centers can provide participants with the following:

 

  • Assistance and training in activities of daily living
  • Variety of educational and recreational programs
  • Appropriate nursing and psychological services
  • Full range of therapeutic services including physical, occupational and speech therapies
  • Meals
  • Transportation

A comprehensive clinical team, along with the participant, work together to create a plan of care designed to enhance the individual’s level of function.

Operational Hours:

Monday – Friday — 8:30 AM – 4:00 PM (except major holidays)
